The family of Barry Kozak of Theodore, beloved husband of Connie Kozak sadly announce his passing on December 25, 2024. Barry was 72 years of age.
A Funeral Service will be held on Saturday January 4, 2025 at 2:00 p.m. from the Yorkton Memorial Gardens Family Centre.

Interment will follow in the Theodore Town Cemetery.
Memorial donations in memory of Barry Kozak may be made to the Heart & Stroke Foundation or Diabetes Canada as gifts of remembrance.
